BTS Health Digest #92 - Can I Switch Without Going Back to Being a Student?

For most of the well-evidenced routes, yes. Here is where retraining is and is not needed.

The fear that changing means years back in a classroom, and back at the bottom of a pay scale, keeps a lot of people exactly where they are. For most of the routes worth taking, that fear is misplaced. The strongest evidence in our data points to roles you reach through a secondment, an internal move or a lateral transfer, not a fresh degree.

How people actually progress in these roles

One of the clearest signals in our data is what it takes to move up in these roles once you are in. It is almost never a qualification. It is demonstrable experience.

For a Band 7 Project Manager, our most authoritative mapping (16 job packs across 12 trusts), progression to Band 8a is described as requiring demonstrable experience leading large-scale programmes, not a master’s. For a Band 6 Clinical Research Nurse, our best-evidenced nursing route (an established mapping across 9 packs), progression to Band 7 asks for leadership of a research portfolio and study set-up experience. The currency of advancement here is track record, which means the way in is track record too.

Routes you can reach without retraining

The common thread is the secondment. Across these routes, the standard on-ramp is to do a piece of the work while still in your current role, so you build the track record that makes you a credible applicant. No enrolment required.

Where retraining genuinely is needed

Honesty matters, so here are the routes where a switch really does mean acquiring new technical skills. Software development and engineering need genuine technical ability. Senior data science needs tools like SQL and sometimes Python, which for most clinicians means real study. Some specialist roles ask for a professional qualification as an entry gate. If one of those is your target, plan for the learning. For the majority of the well-evidenced routes above, though, you are transferring experience, not starting over.

On the money, the reassurance is real: these moves are usually lateral or a step up rather than a drop, and our pay guide sets out the figures by band on the current 2026/27 Agenda for Change scale. If you want the fuller picture on timing and mid-career feasibility, our piece on whether it is too late at 40 and our guide to portfolio careers cover it.
